Surgery Instructions || Notice of Privacy Practics We accept most major insurances and are a "preferred provider" for many plans. Please be sure to bring all insurance cards to your appointment so we can file the claim for you. We accept Visa, MasterCard, Discover and American Express as well as cash and personal checks. Please arrive early to your first appointment to allow time to complete your medical history and demographic forms. We also maintain a large selection of informative videos on all aspects of our urology practice.
